The Center for Urban Community Services’ Training and Consulting Services(TCS) is seeking a full-time Consultant.  TCS helps human service organizations across the nation through staff training, hands-on coaching, facilitation for planning processes, and assistance developing housing and service programs that have a measurable and meaningful impact. TCS helps agencies to implement evidence-based practices, develop systems to monitor and strengthen program outcomes, and start-up new programs.
 
Responsibilities:

 

  • Leading and/or serving on training and consulting project teams locally and nationally, includes:

Qualifications: The ideal candidate will have a commitment to and passion for helping human service organizations to build programs that have a measurable and meaningful impact. Specific requirements include:
§  Master’s degree in public administration, social policy, public health, social work or other related field, including at least two years of supervisory experience
§  Minimum of five to seven years of experience in human services, including at least one year of experience in the homeless services sector

Neighborhood Coalition for Shelter (NCS)
 
Position: Senior Case Manager, Louis Nine House
 
Responsibilities: The Senior Case Manager is responsible for supporting and advancing the program goals of the Louis Nine House by building a clinical team that utilizes a strong multi-disciplinary approach to service provision. In addition to carrying a caseload, s/he ensures that each resident works with a Case Manager to create a Service Plan and uses the document to monitor goals and outcomes.  Quality Assurance, consistent with professional/funding standards is also a major function of this position.
·         Supervise two Case Managers and collaborate with Vocational Rehabilitation Counselors to ensure comprehensive service provision.
·         Monitor intakes of all new residents and promptly assign them to a Case Manager.
·         Ensure that each resident works with their Case Manager and a Vocational Rehabilitation Counselor to develop an Action Plan that includes time-defined  goals based on their specific service needs to develop life and work skills.
·         Provide advocacy and referrals for entitlements, legal assistance, medical and other emergency needs, as well as referrals for on-going follow-up needs.
·         Work collaboratively Youth Development Coordinator and residents to develop activities that will strengthen daily living skills.
·         Carry a case load and provide individual and group counseling, as-needed basis.
·         Quality Assurance: ensure that staff accurately record all resident contacts including services provided by on-site psychiatrist.
·         Examine all client charts for proper paper charting on a regular basis for accuracy and professional standards.
·         Participate in and/or facilitate case conferences, staff meetings, training sessions and community meetings.
·         Work cooperatively with staff at all levels to ensure quality service provision and the development and improvement of team performance.
·         Other duties assigned.
 
Qualifications:
 
·         Licensed Masters in Social Work or human service related field with 3-4 years progressive supervisory experience.
·         Qualified candidate must have working knowledge of SPMI/SED population, preferably a setting with young adults; and a strong understanding of the developmental needs of young adults/at-risk youth; a sensitivity to ethnic and cultural diversity; and a knowledge of and a commitment to working from a strength-based perspective.
·         Must have excellent written and verbal communication skills.
·         Bi-lingual preferred.
